Employment Changes Form

Section 2.90.075 Orientation. All new employees of elected office holders must have an appointment form (pink slip) processed and signed by the office holder by whom they are employed before payroll will be processed. All other new employees must have an appointment form (pink slip) processed and signed by the County Commissioners before payroll will be processed. The Employment Changes form (pink slip) is to be immediately filed with payroll by Vanderburgh County Department heads when hiring or releasing an employee. The Employment Changes form must be printed and signed by the Office Holder before returning to the Payroll Bookkeeper in the Bookkeeping Department of the Auditor's Office. The form should be turned in before 12:30 PM on the Friday preceding the payroll. When hiring a new employee, the office must schedule an orientation appointment with Leslie Saiko (812-435-5826) to sign up for withholding taxes and other benefits. All pink slips must be turned in to payroll and the employee must have attended an orientation meeting before the employee can be placed on the payroll voucher. New employees also need to be scheduled to start work at the beginning of their pay period.
